Comparison of EU5 hospital market plus 10 largest European operations

This week we take a closer look at how the hospital market stacks up, specifically in five major European markets – France, Spain, Germany, Italy and UK. Here we compare the total, and for profit and public/not-for-profit market revenues, and look at where the international operators have the largest revenues. These figures are a snapshot of the position based on revenues in 2020 taken from our hospital report in HBI Intelligence and the operator database.

The for-profit market accounts for around 13% of the total market in hospitals, which is unsurprising as most countries tend to lean toward public provision.

The UK has the largest total market, the largest public sector and the smallest share of the for-profit. This is unsurprising as the UK sector is dominated by the NHS, yet one of the featured top ten is a UK based for-profit provider.

France has the second largest total market, the smallest public/not-for-profit, but third largest for-profit. This is predominantly due to how the public health and insurance systems work. Ramsay Sante and Elsan both play a large role in French hospitals.

Germany has the third largest market overall, the second largest public market but the largest for-profit market. Despite being third largest overall, four of the groups featuring in our top ten are German, with a special mention to Fresenius Helios the largest by revenue.

Italy is the next largest market and sits in the middle in each category. Just one of our top ten comes from Italy, although there are a few other Italian groups not far behind. Spain is the smallest market overall and also is the second smallest market in comparative terms within each category, but again one of the top ten is a Spanish hospital group.
