What is the nature of the low-cost service that the nominated organisation is delivering and to whom?

“Dignio Remote Care” is a combination of Remote Patient Monitoring and Telehealth with a unique preventive methodology given to patients with chronic conditions, such as Diabetes, COPD, Hypertension and Cardiovascular conditions. Patients are cared for by specially trained nurses in a Remote Care Center, and they use a tablet combined with medical devices to perform a simple health assessment on a daily basis. The goal is to “coach” the patients to become more knowledgeable about their chronic condition and take more responsibility for their own health. We also help patients with medication compliance, using electronic medication dispensers connected to our Chronic Diseases Management Platform, Dignio Prevent. The service has primarily been provided to seniors so far, in order to save cost on the traditional (and more resource demanding) healthcare services in a “Remote Primary Care-fashion”.
