SPEAKER CONFIRMATION AND RELEASE

This Speaker Release (“Release”) is provided to Healthcare Business International (HBI) with respect to HBI’s London conference (“the Event”) on the dates and at the location noted above.
By signing below, I acknowledge that I have read and agreed to the following:
1. The rights set forth in this Release are granted as a condition of, and as consideration for, HBI permitting me to appear at the Event and/or promoting my appearance in connection with the Event and/or other consideration, the sufficiency of which is hereby acknowledged. I grant these rights without requiring other compensation or remuneration of HBI.
2. I have full authority to grant the rights set forth in this Release.
3. I acknowledge that HBI does not provide any compensation, travel and/or expense reimbursement related to this speaking engagement.
4. HBI or its agents may record and transcribe my speech, remarks, comments, responses, and image in connection with my presentation or any interview at the Event, on any media, whether digital, analogue or print (“Presentation Recording”). HBI or its agents also may reproduce any visual aids that I provide to HBI in connection with my presentation, including without limitation PowerPoint slides, papers, etc. (collectively, “Presentation Materials”).
5. HBI and its agents may use, reproduce, distribute to the public, broadcast, transmit, display, create derivative works from, or otherwise publish throughout the world the Presentation Recording and Presentation Materials, as well as my image, voice, likeness, and biographic or other identifying information (including without limitation my company or organisation, title, etc.) used in connection with the Presentation Recording or Presentation Materials, or other publicity, promotion, or commercialisation of the Event.
