NHS to facilitate integrated care

The NHS in England prides itself on the strength of its primary care. This service is set to become the backbone in the delivery of integrated care with primary care doctors coming together with pharmacies, care homes, hospitals and wider social care. The aim? To deliver better care that encourages prevention and patient recorded outcomes. So far the movement feels a little hobbyist.
